Gepants block the CGRP receptor. CGRP is an important signalling molecule in migraine biology, but blocking it does not mean every person will respond or that CGRP explains every feature of migraine.
First establish whether the clinician prescribed rimegepant for an attack or for prevention. Acute response and changes in monthly migraine burden are different measures. Do not judge a chronic preventive effect from an acute-treatment rating.
Before and after a treatment change, record migraine-like days, total headache days, acute medicine days, severity and functional impact. If you notice side effects, record what happened and when rather than relying on memory at the next appointment.
